ANTHONY M. KOZAK, age 75, beloved husband of Ann Marie (nee Keffler) for 45 years, loving father of Michael Kozak (Katie) and Sarah Kaplan (Phil), dearest papa of Jack Anthony Kozak, son of the late Stanley and Olga, brother of Alex and Nancy, uncle and friend of many. In lieu of flowers, the family suggests contributions to Holy Family Church or Holy Family Hospice. Private graveside service at Holy Cross Cemetery. Tony was long time parishioner and lector at Holy Family Church where a Memorial Mass will be announced at a later date.
